SiamSculpt (Consulting) LLC Cancellation & Amendment Policy
1. DEPOSIT
1.1 Upon booking, a non-refundable deposit is required. The final payment must be settled no later than 6 weeks prior to the scheduled surgery date.
1.2 The deposit may be utilized as a one-time contribution towards a procedure valued at more than $4,990 USD, booked through SiamSculpt, with a surgery date within 24 months of the original Deposit Paid Date, provided the procedure is available at the initially booked hospital.
1.3 Surgery Deposits are non-refundable if you are unable to attend your scheduled surgery date due to government-imposed travel restrictions, government-mandated medical restrictions, or personal reasons, whether related or unrelated to COVID-19. However, your Surgery Deposit can be applied as a credit towards a new SiamSculpt surgery date within 24 months from the payment of your original deposit.
2. AMENDMENT AND CANCELLATION FEES
2.1 If the client amends or cancels more than 30 days before the Confirmed Surgery Date, an Amendment fee of $150 or a Cancellation Fee of $350 will apply.
2.2 Amendments or cancellations made by the client within 30 days of the Confirmed Surgery Date will incur an Amendment fee of $250 or a Cancellation Fee of $500.
2.3 Should the client make amendments or cancellations within 7 days of the Confirmed Surgery Date, an Amendment fee of $300 or a Cancellation Fee of $1000 will be imposed.
2.4 If the surgeon cancels the surgery during or after the consultation and before the Confirmed Surgery due to the client’s non-disclosure of medical history, medications, or non-compliance with pre-surgery SiamSculpt Guidelines and Conditions, a $2000 Cancellation Fee and an Administration Fee of $500 will be levied. Additionally, all incurred costs, including medical tests, surgeon consultations, hospital inpatient accommodation, and transfers, will be the client’s responsibility.
2.5 If the surgeon or the Medical Team cancels the surgery in the interest of patient safety (not due to the client’s non-disclosure), no Cancellation Fee will apply. However, an Administration Fee of $500 and all associated costs, including medical tests, surgeon consultations, hospital inpatient accommodation, and transfers, will be charged.
2.6 Cancellation by the client after the consultation with the surgeon and before the Confirmed Surgery will incur a $1000 Cancellation Fee, an Administration Fee of $500, and all related costs.
2.7 The Deposit cannot be applied to Amendment and Cancellation Feess
2.8 It is strongly recommended to obtain a suitable Travel Insurance Policy with coverage for Cancellation Fees.
3. PROCESSING REFUNDS
4.1 To initiate a refund, clients must furnish their bank account details in writing via email to SiamSculpt. Upon receipt of the correct bank account details, the refund process may take up to 7-10 business days. An incorrect submission of account details, resulting in the need for re-processing, will incur a fee of $25 USD.

If your inability to travel stems from government-imposed travel restrictions or mandated medical directives, your surgery date can be deferred without incurring any administration or cancellation fees. It’s important to note that if government restrictions are in effect at the time of booking and persist, necessitating adjustments or cancellations to your surgery, SiamSculpt requires a minimum of 30 days’ notice to make such changes without imposing a fee. If government restrictions are enforced within 30 days of your scheduled surgery, you will not be subject to an administration or cancellation fee. The applicable fee is $150 USD. Please be advised that SiamSculpt does not assume responsibility for informing clients of current government travel restrictions; it is the client’s responsibility to stay informed by consulting relevant government-provided resources.
